Wellness Check-ins
Staying Connected, So No One Is Forgotten
We provide structured, compassionate outreach for individuals facing high levels of vulnerability, including mental health challenges, substance use, isolation, and unmet medical needs. Our check-in process includes:
- Identifying and engaging individuals in visible encampments or high-risk areas
- Building trust through consistent, respectful contact
- Tracking wellness over time with a confidential outreach log
- Responding to crisis signs such as overdose risk, suicidal ideation, or severe illness
- Making timely referrals to housing, recovery, mental health, and medical services
Check-in frequency is based on individual risk level, with high-priority visits every 2-3 days. Each visit includes basic wellness questions, supply distribution, observation of changing conditions, and ongoing emotional support. We celebrate progress, offer assistance without conditions, and maintain long-term relationships rooted in respect.

Crisis Transportation Services
Safe, trauma-informed transport during moments of urgent need.
When individuals are in crisis and ready to seek help, we offer transportation to critical services—removing barriers and connecting them to safety, care, or recovery.
Crisis transport may be offered when someone is:
- Experiencing a mental health emergency or suicidal thoughts
- At immediate risk of overdose or recently overdosed
- In unsafe or violent conditions (e.g., assault, trafficking, domestic violence)
- Suffering urgent but non-ambulance-level medical issues
- At risk due to extreme weather exposure
- Ready to access detox, treatment, or housing services
All transportation is provided with verbal consent, nonjudgmental support, and safety protocols (such as traveling in pairs and confirming drop-offs with partner agencies). We act quickly, compassionately, and without requiring behavior change—because no one should have to face a crisis alone.
